Aftercare planning in Los Angeles
The Blackburn Group helps people in Los Angeles plan for life after addiction or mental health treatment. We put together case management, therapy, monitoring and mentoring so the first months at home have structure and support. Call (888) 305-5422 to start planning, ideally before discharge.
Leaving treatment is a big step. The daily schedule, the support and the people who were there every day suddenly go away. Old stress, old places and old routines are waiting. The first weeks and months back home are often when people need the most help, and when they are least likely to have a plan.
Aftercare planning is part of Blackburn Recovery Services, one area of service within The Blackburn Group. Because clinical care, monitoring and mentoring are part of the same organization, the plan can be put together in one place and adjusted as life changes.
What happens when you call
The best time to call is before treatment ends, but it is never too late.
Here is what usually happens when you call (888) 305-5422:
- A real person answers or calls you back, and listens first. You do not need the right words or a diagnosis.
- We ask a few simple questions to help sort out what is going on. We want to know who needs support, what has changed, what has already been tried, and what feels most urgent.
- We point you to the right team inside The Blackburn Group, or to an outside resource if that fits better.
- Nothing is shared with anyone without consent. If the person you are worried about is an adult, we talk about what they have agreed to share.
For aftercare questions, we will ask about the treatment program, the expected discharge date, what the program recommends, where the person will be living and who is involved. With consent, we can coordinate with the treatment team so nothing is lost in the handoff.
Who we help
We help people coming home to Los Angeles and the families supporting them.
- Adults finishing residential, outpatient or hospital-based treatment.
- Young adults coming home who need structure, direction and accountability.
- Professionals returning to work who want a private, steady plan.
- People who have been through treatment before and want this time to go differently.
- Families who want to know how to help without taking over.
Our guide to the first 90 days after treatment explains what this stretch often looks like.
How we work
A good aftercare plan is specific. Who, what, when, and what happens if things slip.
Depending on the person, an aftercare plan may include:
- Case management. One steady person who keeps appointments, providers and next steps organized, and keeps the family informed with consent. See case management.
- Therapy and psychiatry. Individual therapy, plus psychiatry and medication management, through Blackburn Clinical Services.
- Drug and alcohol monitoring. Random testing with electronic notice, home testing and lab confirmation, with results shared only with consent. See drug and alcohol testing for families.
- One-on-one mentoring. A dedicated mentor to help rebuild routines around work, school, health and relationships. See mentoring.
- Family support. Family coaching and a free virtual family support group every two weeks, open to any family.
The plan also names early warning signs and what everyone agrees to do if they show up.
When things get hard
Slips and hard weeks happen. A plan makes them easier to handle.
Knowing the warning signs of relapse helps everyone act early, before a hard week turns into a crisis. If someone is returning to a demanding job, our guide on returning to work after treatment can help. When mental health and substance use overlap, read about co-occurring disorders.

Questions
When should aftercare planning start?
Ideally a few weeks before treatment ends, so the plan is ready on day one at home. If someone is already home, it is still worth calling. It is never too late to put structure in place.
Doesn’t the treatment program handle aftercare?
Many programs make recommendations. An aftercare plan turns those into real appointments, people and routines in Los Angeles, and keeps them going after the program’s role ends. With consent, we work with the program.
Is monitoring required?
No. Monitoring is one option. Some people choose it because it adds structure and helps rebuild trust with family or an employer. Results are shared only with the person’s consent.
Can family be involved?
Yes, with the person’s consent. Family coaching and the free virtual family support group every two weeks help families support recovery without taking over.
How do we get started?
Call (888) 305-5422. A real person will listen, ask about treatment and timing, and help you start building a plan.
